Medical Authorization Form - any students who are required to take medication while at school must have the following form fill in by a parent/guardian. All medication is to be kept in the school office and administered by office staff.

Driver Authorization Form - any parent/guardians or staff who are transporting students in their personal vehicles are required to fill out the following form prior to doing so. Parent/guardians are also required to have a criminal record check submitted to the office.

Criminal Record Check Form/Letter - having this form/letter in hand when attending the Regina Police Station in person will save the cost of the criminal record check. Please contact the school office to request a copy.
